Choose Your Starting Point

Start by uploading an existing PDF, clicking 'Create Blank PDF' for a new document, or choosing a pre-built template to get started quickly with common form layouts.

Add Form Fields

Select a field type from the toolbar — text, checkbox, radio button, dropdown, button, or signature. Then click on the PDF canvas to place the field. Drag to reposition and use the corner handle to resize.

Configure Field Properties

Select any field on the canvas to edit its properties in the side panel. Set field names, labels, default values, dropdown options, and mark fields as required.

Generate and Download

When your form is ready, click the Create Form button. The PDF will be generated with all interactive AcroForm fields embedded. Choose to flatten fields if you want them to be non-editable.

Important Notes

This form creator uses pdf-lib to generate standard AcroForm fields that are compatible with Adobe Acrobat, Preview, Chrome PDF viewer, and most PDF software. Text fields support single and multi-line input. Radio buttons with the same name form a group where only one option can be selected. The flatten option permanently converts form fields into static content — use this only for final distribution. All processing is client-side — your files never leave your device.
